Biddeford Animal Hospital offers select form(s) online so you can complete it in the convenience of your own home or office.

• If you do not already have AdobeReader® installed on your computer, Click Here to download.
• Download the necessary form(s), print it out and fill in the required information.
• Fax or mail us your printed and completed form(s) or bring them with you to your appointment.

New Patient Registration Form - Required for new clients.

If you are new to our hospital use this form to give us enough information to create a record into our hospital management software so that we can effectively and efficiently provide the service you and your pet deserve!  If you have more than two pets just print extra copies as needed.  You can bring these forms with you to your scheduled appointment or you can mail or fax them in ahead of time to (207) 286-9503 and we will then have the information put in the software before you arrive.

Records Release Authorization Form - Optional

This form can be filled out so we can request for you a copy of your pet(s) medical records from your previous veterinarian.  Please feel free to print off a copy then fill in the hospital we are to request the records from, your name, your pets names and be sure to sign and date at the bottom.  You can mail us this form, fax it to us at 207-286-9503, or bring it with you.  If you have the ability to get it to us before your appointment this will allow us time to possibly have the record available during your appointment.

Website Member Topics Registration Form - Optional

This form can be filled out to register for access to the member section of our website. You can also sign up for our monthly newsletter to keep up on current health issues and news and events in our office. You can print it out and bring it in to our office or Click Here to register online! The online newsletter sign-up is also on the right.
